How Is Rhinoplasty Performed?

Rhinoplasty is considered one of the most technically demanding plastic surgery procedures, and it’s important to choose a board-certified plastic surgeon who is very familiar with the procedure and the nose’s delicate anatomy. There are two primary approaches to the procedure: open and closed rhinoplasty. Dr. Rosenthal typically uses the open rhinoplasty technique. He uses general anesthesia to keep patients comfortable throughout the surgery. Rhinoplasty surgery typically takes about 2 hours.

Open Rhinoplasty

When performing open rhinoplasty, Dr. Rosenthal starts by making a small incision on the strip of skin dividing the nostrils, called the columella. After making the incision, he gently lifts the skin off the nose to expose the nasal bone and cartilage. He makes adjustments to these to produce the desired results. The scar resulting after an open rhinoplasty is very small and is usually imperceptible after the incision heals.

Closed Rhinoplasty

The difference between an open and closed nose surgery is that Dr. Rosenthal doesn’t make an external incision during closed rhinoplasty. Instead, all incisions are made within the nose, and he works through these incisions to change the shape or size of the nose. The drawback of closed rhinoplasty is that the surgeon doesn’t have a direct visualization of the bone and cartilage.

Why Do People Get Rhinoplasty?

People consider getting rhinoplasty—often called a nose job—for various reasons. Most of our Boynton Beach patients have thought about changing the size or shape of their noses for a long time. In some cases, a person may have difficulty breathing through their nose and may need septoplasty to clear their airways.

The primary reasons for cosmetic rhinoplasty include:

  • Minimizing a prominent bump or hump on the bridge
  • Narrowing a bulbous nose to create a streamlined nasal tip
  • Elevating a droopy tip or bringing down an upturned tip

A teenager considering rhinoplasty should be old enough that their noses are fully developed. For girls, that occurs at about age 15. Boys typically should wait until they’re 17.

What Is Revision Rhinoplasty?

Dr. Rosenthal often performs revision rhinoplasty for patients who are unhappy with the results of nose surgery performed at another practice. The revision surgery poses unique challenges because of the presence of scar tissue that develops following the original procedure and, in some cases, insufficient cartilage. Dr. Rosenthal may use cartilage from elsewhere in the body, particularly the ear and occasionally the rib. The cost of revision (secondary) rhinoplasty ranges from $19,596 to $24,596.

What Happens During Rhinoplasty Recovery?

It usually takes at least 10 days to 2 weeks for the initial healing process after rhinoplasty. During that time, expect swelling, bruising, and a stuffy-nose feeling (but resist the urge to blow your nose). However, patients are often pleasantly surprised to find there isn’t much pain after nose surgery.  

To help reduce swelling as quickly as possible, you should keep your head elevated while you sleep or rest. You should also gently apply ice or cold compresses during the first 48 hours after surgery and avoid taking ibuprofen (Advil® or Motrin®) for a week.  

It’s important to realize that even after the initial recovery phase is over, residual swelling will prevent you from seeing the ultimate outcome of the surgery for several months to a year.
